Pilates Sculpt

Shea.Moves LLC provides instructor-led group fitness classes that may include strength training, mobility, Pilates, and general wellness activities in a group setting. I understand that classes may vary in format, intensity, and structure.

I acknowledge that participation in group fitness classes involves physical activity and inherent risks, including, but not limited to, muscle strains, sprains, falls, injuries, or other health complications. I understand that it is my responsibility to inform the instructor, prior to participating, of any medical conditions, injuries, surgeries, illnesses, or physical limitations that may affect my ability to safely participate.

I understand that the instructor is not a licensed medical professional and that these classes are not intended to diagnose, treat, or replace professional medical advice, care, or treatment. Any personal or health information I choose to disclose will be kept confidential unless disclosure is required by law.

I voluntarily assume all risks associated with participating in these classes and confirm that I am physically able to participate. I hereby release and discharge Shea.Moves LLC and Shea Puma from any liability for injuries, damages, losses, or claims arising out of or related to my participation in any class, whether conducted on-site, off-site, or virtually, to the fullest extent permitted by applicable law.

I have read, understood, and agree to all terms outlined above.

Class cancellations must be made at least 2 hours before class. If Shea manually marks me as a no-show, I authorize Shea.Moves LLC to charge the payment method used for this booking a separate, non-refundable $10 no-show fee in addition to the original class payment.

I understand that photographs and/or video recordings may be taken during classes or events hosted by Shea.Moves LLC. I grant Shea.Moves LLC permission to use my likeness, image, voice, and appearance in photographs and/or videos for promotional, advertising, editorial, educational, and social media purposes without compensation or further approval.

If I do not wish to be photographed or recorded, I understand that it is my responsibility to notify Shea.Moves LLC prior to the start of the class or event, and reasonable efforts will be made to honor my request.
